60-Day Pilot Program Agreement

Between Rewire App LLC ("Rewire") and ("Facility"). This agreement covers a free 60-day pilot of the Rewire between-session clinical platform at the Facility.

Pilot terms

What Rewire provides

• Full platform access for the pilot cohort — client app, clinician portal, and facility admin dashboard
• White-glove setup: Rewire creates the facility workspace and onboards both clinicians in a 30-minute training call
• Direct support channel for the duration of the pilot
• A signed Business Associate Agreement (BAA), executed before any patient is onboarded
• A written outcomes summary at the end of the pilot: engagement rates, between-session activity, and distress-trend movement across the cohort

What the Facility provides

• Two clinicians who will assign between-session exercises as part of their normal care
• Up to ten patients appropriate for the platform, onboarded within the first 30 days
• A 30-minute midpoint check-in call and a 30-minute close-out call to review the outcomes summary

Data & privacy

• Patients own their data. Each patient's account, session history, and journals belong to the patient and remain theirs after the pilot ends — regardless of whether the Facility continues with Rewire.
• Clinicians own their clinical notes. Rewire claims no rights over clinical content created during the pilot.
• Rewire may reference anonymized, aggregate pilot outcomes in its own materials. The Facility's name is never used without written permission.

At the end of the pilot

• Rewire presents the outcomes summary on the close-out call
• The Facility may convert to a paid plan (per-bed or flat facility pricing), decline, or extend the pilot by mutual agreement
• If the Facility declines: clinician and admin access ends, and patients keep their accounts and data with the option to continue individually
